Welcome to the Bolton High School Registration page. Parents wishing to register a student for grades 9-12 should submit their registration documents via email to guidance@boltonct.org , fax (860-645-8374), or mail (72 Brandy St. Bolton, CT 06040). If you have any questions, please feel free to contact the guidance department at (860-649-3353). When forwarding registration documents, please include the following:

• Registration Packet
• Health Assessment Record
• Withdrawal Form
• Verification of student’s age (to include but not be limited to):
  • Photocopy of child’s birth certificate
  • Earlier school records
  • State issued identification documents
  • Parent’s affidavit or unsworn statement as to student’s age
  • Physician’s certificate verifying student’s age
  • Immunization Records
• Proof of parent’s residency (see Bolton Residency Requirements)
• Student Residency Affidavit (if applicable)
• Certificate of Residency (if applicable)
• Free and Reduced Lunch Application
